We must have a written or oral prescription to take any x-rays.
Please bring your prescription slip to your appointment.

Payment is required when services are rendered. 
Insurance is not accepted as payment.

Please have hair combed and teeth brushed for photographs.
You may also be asked to remove earings, necklaces, retainers, etc.

Please notify us in advance for any disability requiring special assistance.
We may need to schedule additional time for certain procedures.

Patients more than 10 minutes late may be asked to reschedule.
Please call if you know you will be late.

24 hour prior notice of cancellation is appreciated. We do not over-book. 
Your early cancellation may help another person who needs your time.
